29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:    a) On or about 04/26/19 in the Facilities building; employees were exposed to a fall hazard while using the stairway from the first floor to the lower floor which was 39 inches wide with six risers and five treads and one open side and had only one hand rail on the enclosed side when Table D-2 requires a hand rail on the open side.
