At 24211 North West Service Road, Warrenton, Missouri, specifically inside of the Production Area, the solid rubber tires belonging to the Komatsu Fork Lift, m/n FG40ZT-8, s/n 135043A, were missing large chunks of rubber.  The right front tire had a two inch wide piece of steel imbedded in the tread.  This violation was noted on 04/30/19.
