29 CFR 1910.219(m)(2)(i): Expanded metal, sheet or perforated metal, and wire mesh was not securely fastened to a frame. (b) Production - On April 29, 2019, the employer failed to ensure that hazards created by the Air Chicago Towel Folder #1 rotating feeding ribbons were guarded. A guard meant to cover the top of the unit was missing. (c) Production - On April 29, 2019, the employer failed to ensure that hazards created by the Air Chicago Towel Folder #4 rotating feeding ribbons were guarded. A guard meant to cover the top of the unit was missing. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Production Area- On April 26, 2019, and on May 9, 2019 the employer failed to establish a Lockout Tagout Program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on machines, such as but not limited to the Air Chicago Towel Folding Machines, Milnor Wash Line Machines, where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative. b) Towel Folding Machine - On April 26, 2019, the employer failed to establish a Lockout Tagout Program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed unjamming of machines, such as but not limited to the Air Chicago Towel Folding Machines, where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment was isolated from the energy source and rendered inoperative. An employee suffered an occupational injury while attempting to unjam the Air Chicago Towel Folding Machine on April 26, 2019. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
