29 CFR 1926.20(b)(2): The employer did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons: a) Job site, 2019 Brownsville Road, Pittsburgh, PA: On or about April 24, 2019, during roofing activities, the employer did not ensure that an appropriate system was implemented to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, employees tying off to a knotted strap at waist level on the front of harnesses for fall protection, failing to properly extend a ladder at least 3 feet above the roof line, employees without appropriate head protection in use were working in areas where materials above could fall on them and harnesses used for fall protection were not properly sized/worn/fitted.
