29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces: JOBSITE: The Werner 24-foot orange extension ladder which was placed against a brick wall and used by an employee to remove scrap metal from the wall was neither secured nor lashed to prevent from displacement while set on a slippery concrete floor. On or about 04/11/2019, the above ladder skid out from the bottom and fell to the floor while the employee was performing the above task. The employee sustained a severe lacerations to his head and nose, and a severely sprained or broken right ankle.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired. JOBSITE: The Werner 24-foot orange extension ladder from which an employee was working was missing a foot tread and not removed from service.
