West end of the residential house - On or about April 24, 2019, an employee did not wear a protective helmet when lifting sheets of plywood over his head, thus exposing the employee to a struck-by hazard.

Roof of the residential house - On or about April 24, 2019, employees operated a pneumatic nail gun without the use of eye protection, thus exposing employees to struck-by hazards.

Roof of the residential house - On or about April 24, 2019, employees worked from the roof while installing plywood sheathing without the use of a fall protection system, thus exposing employees to a fall hazard of approximately 10.7 feet.  b). Back porch - On or about April 24, 2019, an employee worked from a porch beam while installing plywood sheathing without the use of a fall protection system, thus exposing the employee to a fall hazard up to approximately 9.6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow for the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device such as a grabrail was not provided to assist employees in mounting and dismounting the ladder.  a). West end of the residential house - On or about April 24, 2019, a wooden job-made ladder was not secured to a rigid support and a grasping device was not provided, thus exposing employees to a fall hazard of approximately 10.7 feet.
