29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. a). At the worksite - On or about and times prior to April 24, 2019, personal fall protection equipment was improperly used, in that two employees were connected to one lanyard when working from the work platform of a JLG Telescopic Boom Lift (model: 460SJ), thus exposing employees to a fall hazard up to approximately 16 feet.

At the worksite On or about and times prior to April 24, 2019, the employer did not ensure employees were trained by a competent person on the proper usage of a Miller Titan II Double-Leg Shock Absorbing Lanyard (model: T5122), thus exposing employees to a fall hazard up to approximately 16 feet.
