29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. A) Fechko Excavating, LLC on or about April 24, 2019, at the intersection of Center Ridge Road and Lear Nagel Road in Avon, Ohio: The employer failed to ensure employees were adequately protected from cave-ins when working in a trench with a depth of 6 feet, while taking measurements under a fiber optic bank in preparation for a water pipe installation.
