29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: Location: 250 Carrollwood Drive Tarrytown NY 10591 a) On or about April 29th, 2019, employees were renovating a residential roof without the proper fall protection and an improperly erected ladder jack scaffold thus exposing the employees to a fall hazard.

The Company Twin Brothers Roofing Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1091291, citation number 1, item number 3 and was affirmed as a final order on November 10, 2015, with respect to a workplace located at 40 St Roach Ave Greenwich, CT 06830.
