29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On or about 4/24/19, on a roofing jobsite, at the Clarence First Presbyterian Church, 9675 Main Street, Clarence, NY: Ladder Jack Scaffolding had been erected on the side of the church running parallel to Main Street. Employees were on the scaffolding and engaged in roofing work.  The employees were exposed to a fall of approx. 14 ft. because fall protection was not being utilized. Roof anchors & lifelines were in place on the roof, and hanging over the scaffolding, but employees were not tethered to them.       NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged one to another:        a) On or about 4/24/19, on a roofing jobsite, at the Clarence First Presbyterian Church, 9675 Main Street, Clarence, NY: Ladder Jack Scaffolding had been erected on the side of the church running parallel to Main Street. Employees were on the scaffolding and engaged in roofing work. The employees were exposed to a fall of approx. 14 ft. because scaffold piks were bridged to one another.         NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) On or about 4/24/19, on a roofing jobsite, at the Clarence First Presbyterian Church, 9675 Main Street, Clarence, NY: Employees were engaged in roofing work on the steep roof of the church on the side running parallel to Main Street. The employees were exposed to a fall of approx. 16 ft. because fall protection was not being utilized. Roof anchors & lifelines were in place on the roof, but employees were not tethered to them. Pitch of the roof was approx. 12 in 12.      NO ABATEMENT DOCUMENTATION REQUIRED     AVA Roofing & Siding Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1379546, citation number 1, item number 1 and was affirmed as a final order on 4/1/2019, with respect to a workplace located at 181 West Avenue, Lockport, NY 14094.    AVA Roofing & Siding Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1289871, citation number 1, item number 1 and was affirmed as a final order on 2/21/2018, with respect to a workplace located at 110 Highgate Avenue, Buffalo, NY 14214.
