29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about April 29, 2019 and at times prior thereto, at a workplace located at 2475 Esters Blvd. #100, Grapevine, Texas; in the production area, employees operating Fillet Choppers were exposed to amputation hazards. Adequate machine guarding was not in place to prevent exposure to point of operation hazards.
