29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). At the establishment, in the warehouse, employees operating the Toyota 6000 pound forklift, model 8FG35U, serial number 12054 and the Toyota 4500 pound forklift, model 7FGU25, serial number 70687, were not trained or certified by the employer in the safe operation of the forklift, exposing employees to the hazard of being struck by the lift.
