29 CFR 1910.268(g)(1): The employer did not ensure the proper use of a positioning system or a personal fall arrest system while work is performed at positions more than 4 feet (1.2 m) above the ground on poles and on towers: On or about April 17, 2019, at the jobsite, the employer did not ensure that safety belts and straps were used when work was performed above 4 feet aerial lift exposing employee to a fall hazard.
