29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) At the above referenced inspection site, on or about April 22, 2019, no fall arrest system or other forms of fall protection were in use by employees who were working while standing on the roof (pitch approximately 6:12), thereby exposing employees to a fall hazard of approximately 11-feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) At the above referenced inspection site, on or about April 22, 2019, the side rails of the portable multipurpose ladder in use by employees to access the roof surface was not extended at least 3 feet above the roof surface, resulting in an approximately 11-foot fall hazard.
