29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided: (a) Reyes Roofing Services, Inc. at 811 Pennsylvania St., Denver, CO 80204: On and before April 12, 2019, the employer did not ensure their employees were protected from injury, in that about four employees engaged in roofing activities on top of a residential home were not utilizing a fall protection system. This condition exposed the employees to a fall hazard of approximately 20-22 feet.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury: (a) Reyes Roofing Services, Inc. at 811 Pennsylvania St., Denver, CO 80204: On and before April 12, 2019, the employer did not ensure that employees were provided eye protection when power tools such as pneumatic nail guns were used during roofing operations. This condition exposed employees to the hazard of projectiles to the eyes.
