29 CFR 1910.219(e)(2)(iii)(a): Completely barred by a guardrail or other barrier in accordance with paragraphs (m) and (o) of this section; or a. At the work site, employees who operated Uni #6 used an open area between the supply rolls and the processing area at the rear of the machine to get from side to side. Use of the walkway exposed employees to moving belts that moved components from rolls, through tension rollers, and into the heat and pressure processing part of the Uni#6 machine. The employer did not ensure an area where employees could pass between belts was adequately guarded.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. a. At the work site, employees who operated Uni #6 were exposed to a caught-in hazard at the rear of the machine. Exposed horizontal shafting the width of the machine was approximately 33 inches from the floor on both sides of the machine.

29 CFR 1910.219(c)(2)(ii): Shafting under bench machines shall be enclosed by a stationary casing, or by a trough at sides and top or sides and bottom, as location requires. The sides of the trough shall come within at least six (6) inches of the underside of table, or if shafting is located near floor within six (6) inches of floor. In every case the sides of trough shall extend at least two (2) inches beyond the shafting or protuberance. a. At the work site, employees who operated Uni #6 were exposed to a unguarded shafting at the rear and underside of the machine. On April 23, 2019, an employee reached into the unguarded area and a finger was amputated by the material around the shaft.

29 CFR 1910.219(c)(4)(i): Projecting shaft ends shall present a smooth edge and end and shall not project more than one-half the diameter of the shaft unless guarded by nonrotating caps or safety sleeves. a. At the work site, employees who operated Uni #6 were exposed to a rotating shaft at the rear of the machine that protruded approximately 4 inches and was approximately 2 inches in diameter. The employer did not ensure that the shaft was adequately guarded to prevent employee exposure to a caught-in hazard. b. At the work site, employees who operated Uni #6 were exposed to a rotating shaft at the rear of the machine that protruded approximately 3 1/2 inches and was approximately 1 1/2 inches in diameter. The employer did not ensure that the shaft was adequately guarded to prevent employee exposure to a caught-in hazard. c. At the work site, employees who operated Uni #6 were exposed to a rotating shaft at the rear of the machine that protruded approximately 10 inches and was approximately 1 3/8 inches in diameter. The employer did not ensure that the shaft was adequately guarded to prevent employee exposure to a caught-in hazard. d. At the work site, employees who operated Uni #6 were exposed to a rotating shaft at the rear of the machine that protruded approximately 4 inches and was approximately 1 1/4 inches in diameter. The employer did not ensure that the shaft was adequately guarded to prevent employee exposure to a caught-in hazard.
