29 CFR 1926.102(a)(1): Eye and/or face protective equipment was not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about April 29, 2019,  during the installation of roofing material at the job site located in Barberton, OH, 44203, the employer failed to ensure that employees working on the roof of a residential property were protected from possible eye injuries from impact or flying objects from other employees installing roofing material. Two employees were  using pneumatic nail guns to secure roofing material to structure without using safety eye-wear or facial protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about April 29,2019, while in the process of completing roofing activities at a residential property located in Barberton, OH, employees were not protected from a fall hazard from a height of approximately 20(+) feet by conventional fall protection and/or devices.
