29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) Location: Mechanization Department. Lockout procedures were not developed and utilized during the calibration machine testing procedures. On March 30, 2019 an employee suffered a finger amputation when the pneumatic actuator closed as the employee reached in to free it from a stuck position. b.) Location: Disc Department Lockout procedures were not utilized during trouble shooting operations where an employee reached in to APRO press to check die and disc. On May 29, 2019 an employee, while checking parts on the APRO press, suffered an injury to a finger, when the operator activated the press and the and the die cycled.
