29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by: (a) Employees were exposed to crushing/pinch point hazards while working on and around the ACME-Gridley 2 5/8" Multi-Spindle Screw Machine (Model No. RB-8 and Serial No. 96169). The employer did not ensure there was adequate guarding to protect employees from rotating gears. The guarding provided to protect employees from the rotating gears were easily removable.
