29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 30, 2019 the employer failed to ensure that effective machine guarding was in pace on the sand mixing apparatus in use at the sand mixing area. Employees are exposed to the spinning parts of the mixer as they reach through the guardrail to add product.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment. On or about April 30, 2019 the employer failed to ensure that unused openings in electrical cabinets were effectively closed as required. The 120v electrical breaker cabinet located in the pattern storage room contained open slots in the panel which were not covered.

29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used where run through holes in walls, ceilings, or floors: On or about April 30, 2019 the employer failed to ensure that flexible cords supplying power to 36v battery chargers and other equipment was not ran through holes in ceilings and walls. Multiple flexible cords were ran through walls and ceilings near the pattern storage area.
