On or about 04/18/2019, 9915 Vista Holly Rd. - an employee was conducting stucco work from the top of an extension ladder that was placed on the top or sixth level of a scaffold, the location of the employee on the ladder higher than scaffold guardrails, exposing the worker to about a 42 feet fall hazard.
