29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  (a) On or about April 30, 2019 - 809 Jordan Lane, Huntsville, AL, an employee was exposed to fall hazards of approximately 25 feet when the employee rode in the wooden trash box to the roof rather than use the ladder on the other side of the church or rather than use a personnel platform designed for use on a rough terrain forklift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) On or about April 30, 2019 - 809 Jordan Lane, Huntsville, AL, an employee was exposed to fall hazards of approximately 6-25 feet when the employee rode in the wooden trash box to the roof which had no guardrail on the front side.  The employee was not using any other type of fall protection while in the box.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:  (a) On or about April 30, 2019 - 809 Jordan Lane, Huntsville, AL, an employee was exposed to fall hazards of approximately 25 feet when the employee unloaded a sheet of plywood from the trash box that had been positioned at the edge of the roof without using fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) On or about April 30, 2019 - 809 Jordan Lane, Huntsville, AL, employees were exposed to fall hazards of approximately 25 feet or more when employees installed shingles on roof using no fall protection or using mountain climbing belts/ropes, and unloaded plywood on the roof after riding to the roof in the wooden trash box strapped to the forklift.
