29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) M+M Inc. at 10909 Irma Drive in Northglenn, CO 80223:  On or before May 2, 2019 the exposing employer did not ensure that each forklift operator was trained to operate the lift. The employer did not train and evaluate each operator who was authorized to operate the forklift to ensure that they were competent to operate the forklift safely. This condition exposed employees to potential struck-by hazards.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  (a) M+M Inc. at 10909 Irma Drive in Northglenn, CO 80233:  On or before May 2, 2019, the employer did not maintain a safety data sheet for each hazardous chemical, lubricants, flammable liquids, and combustible liquids stored in the warehouse. These chemicals include but are not limited to motor oil, propane, wheel baring grease, and gun bore cleaner. This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.

Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.    (a) M+M Inc. at 10909 Irma Drive in Northglenn, CO 80233: On or before May 2, 2019, the employer did not provide information and training to employees in regards to the chemicals used on-site, and on the safety data sheets. 29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) M+M Inc at 10909 Irma Drive in Northglenn, CO 80233:  On or before May 2, 2019, the employer did not ensure that electrical equipment was used and installed in accordance with the manufacturer's instructions.  The employer allowed the use of relocatable power strips that were connected in series (daisy chained) for use in providing power to multiple electrical devices.  This condition exposed employees to electrical shock and fire hazards.
