Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, on or about May 1, 2019 and times prior thereto:  Employees are provided with, and permitted to use, portable dry chemical fire extinguishers.  Employees did not have fire extinguisher training upon hire.    B.)  Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, on or about May 1, 2019 and times prior thereto:  Employees are provided with, and permitted to use, portable dry chemical fire extinguishers.  Employees did not have annual fire extinguisher refresher training.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  A.)  Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, at the north exit door of the vehicle maintenance bay area, on or about May 1, 2019 and times prior thereto:  Employees are provided with, and permitted to use, portable dry chemical fire extinguishers.  A portable dry chemical fire extinguishers was obscured from view by items including, but not limited to, clothing.     B.)  Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, under west table in the vehicle maintenance bay area, on or about May 1, 2019 and times prior thereto:  A portable dry chemical fire extinguishers was on the floor under the table.  The portable dry chemical fire was not mounted, located and identified so that they were readily accessible.  C.)  Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, at the north exit door of the vehicle maintenance bay area, on or about May 1, 2019 and times prior thereto:  Portable dry chemical fire extinguishers were on the floor, and were not mounted to a wall surface.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  A.)  Barbers Chemicals, Inc., 950 Main Street, Sharpsville, PA  16150, on or about May 1, 2019 and times prior thereto:  Employees utilize chemicals including, but not limited to, oils, anti-freeze, and cleaners.  The employer's hazard communication program was deficient in that it lacked elements including, but not limited to:  1.)  Identifying the person(s) responsible for workplace labeling. 2.)  Identifying the person(s) responsible for obtaining/maintaining the SDSs. 3.)  Identifying procedures to follow if it is suspected that the SDS is not appropriate. 4.)  Identifying procedures to follow to determine if the SDS is current. 5.)  Identifying the person(s) responsible for conducting the training.
