The employer did not ensure the guard on a horizontal bandsaw Model RF 1018F was properly positioned to protect employees from hazards created by the unused portion of the blade. This condition exposed employees to lacerations and amputation hazards.

29 CFR 1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion:    (a)BSI Acquisition Corporation dba AmesburyTruth @ 5001 West Delbridge Street, Sioux Falls, SD 57107:  On or about and at times prior to May 1, 2019, the employer did not ensure that the work rest was installed on the Darex drill sharpener series M5, 1/2 HP, 2450 RPM and adjusted to prevent work from being pulled into the wheel which can cause wheel breakage. This condition exposed employees to a struck-by hazard.
