29 CFR 1926.760(a)(1): Each employee engaged in steel erection activity who is on a walking/ working surface with an unprotected side or edge more than 15 feet above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about April 30, 2019, one employee stepped out of the elevated work platform and onto the rooftop of commercial property under construction and never attached to a fall protection system, exposing the employee to a fall hazard of approximately 20 feet

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about April 30, 2019, and at times prior thereto, the employer did not ensure that workers on a metal work platform/basket were trained to recognize unsafe conditions, exposing employees to fall hazards. Quality Steel Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.454(a) which was contained in OSHA inspection number 1175854, citation number 01, item number 003 and was affirmed as a final order on November 14, 2016, with respect to a workplace located at 9811 Vikki Terrace, Austin, TX 78736.
