29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about April 30, 2019 and times prior thereto, the insulation in the following instances had pulled away from the cord allowing wires to be exposed, exposing employees to the hazards of electrical shock:  a) The insulation near the male end of the electrical cords used to power welding stations and hand-held tools. b) The handheld pendant for the North Crane had pulled lose exposing insulated wires.
