29 CFR 1926.34(a): A lock or fastening to prevent free escape from the inside of any building was installed. Building or structure exits were not so arranged and maintained as to provide free and unobstructed egress from all parts of the building or structure at all times when it was occupied.  a) Jobsite entrance, ground level:  A lock was installed on the entrance door which prevented free escape from the inside of the building in case of emergency. On or about 05/01/19.  b) Building entrance, 1st floor:  The only entrance and exit to the building was obstructed by cross bracing of a supported scaffold erected in front of the building. The building exit was not arranged and maintained as to provide free and unobstructed egress from the building at all times when employees were working inside. On or about 05/01/19.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.  a) In front of building:  Employee used electric powered angle grinders to cut bricks. The angle grinders were connected to extension cords, which were routed to a power generator on the 1st floor. The extension cords were missing ground pins. On or about 05/01/19.  b) On supported scaffold in front of building:  An employee was framing the reinforcing metal of the balcony with an electric powered angle grinder. The angle grinder was connected to an extension cord, which was routed to a power generator on the 1st floor. The extension cord was missing a ground pin. On or about 05/01/19.
