29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about April 30, 2019, and at times prior thereto, at the establishment the employer did not establish a Lockout/Tag-out program consisting of an energy control procedure, employee training and periodic inspections in maintenance testing operations. Employees were exposed to unexpected release of energy hazards.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about April 30, 2019, and at times prior thereto, at the service shop a machine having a sprocket wheel and chain drive system which less than 7 feet from the floor was not properly enclosed. The employees were exposed to amputation hazard caused by the unguarded sprocket wheel and chain system.
