Employees were exposed to a caught by hazard while performing servicing and maintenance of paper slitter/rewinder machines as evidenced by, among other conditions; written procedures for lockout and tagout of equipment were not developed and implemented, devices for rendering machines inoperative were not available and/or used, etc.
