29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    a) Workplace, 255 Route 1 & 9, Jersey City,  NJ    The employer did not ensure that the direction of travel was marked approaching  the emergency exit located on the West side of the facility between the data storage room and the material handling and power wash bay area. 29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:    a) Workplace, 255 Route 1 & 9, Jersey City,  NJ    The employer did not document that equipment complied with recognized and generally accepted good engineering practices (RAGAGEP) to include but not limited to IIAR Bulletin 109 (1997), Section 4.9.5 and IIAR 3 (2012), Ammonia Refrigeration Valves, Section 8/8.1a and 8.2a, when  markings such as manufacturer information stamps and plates, that identify the device and their application range or limits,  were obscured or painted over with paint, for the following devices:    a)   The employer did not maintain an over pressure protection relief valve that discharges to the hot gas discharge line, HE-1-06, according to RAGAGEP, because it was painted over by a painting contractor hired to paint closed circuit ammonia system piping located on the roof of the facility.        b)  The  employer did not maintain an isolation valve CB4-02  to isolate the Hot Gas Discharge flow to the AU-13 Evaporator unit circuit piping, in that its identifying manufacturer's plate or stamps were painted over or missing, and that the employer's tag "AU-13, CB4-02" was also missing.          c) The employer did not maintain a solenoid valve CB4-07   that controls the Hot Gas Discharge flow to the AU-11 Evaporator unit circuit piping, in that the employer's tag "AU-11, CB4-07" was missing.       Violation occurred on or about 5/20/2019 to 6/10/2019

29 CFR 1910.147(c)(4)(ii)(B): The employer  did not develop specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    Workplace, 255 Route 1 & 9, Jersey City,  NJ    a) Ammonia Compressors:    For isolating the electrical energy that would engage motors, the employers Lockout Tagout Program procedural steps do not state what electrical device the lock and tag are to be placed, such as a local switch, or on an electrical motor control center or panel, to lockout the Ammonia Compressors.   For isolating the Ammonia chemical energy that could be released, the employers Lockout Tagout procedures  state  that three valves need to be closed to isolate a compressor, and then instructs the employee to refer to other documents, the PSM Operating Procedures and the P & IDs for more information.   It also states to vent any residual ammonia by opening a vent line and bleeding off residual ammonia into a bucket of water or through the pump out vent stack.   However it does not state how to connect the drain line, what type of drain line to use, nor where the compressor drain point or port is located.     b) Ammonia Pumps:     For isolating the electrical energy that would engage motors,  the employers Lockout Tagout Program procedural steps do not state what electrical device the lock and tag are to be placed, such as a local switch, or on an electrical motor control center or panel, to lockout the Ammonia Pumps.   For isolating the Ammonia chemical energy that could be released, The employers Lockout Tagout program procedural steps state that at least five  valves need to be closed to isolate an ammonia pump, and then instructs the employee to refer to the PSM Operating Procedures and the P & IDs for more information.       Violation determined on or about 5-2-2019
