29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about April 25, 2019, employees were exposed to a struck by hazard while installing roofing materials on a new residential home located at 6021 Valley Quail, Seville, Ohio 44273. The employees were using air powered nail guns and the employer did not ensure that they were wearing protective safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about April 25, 2019, employees were exposed to a fall of approximately 11 feet while installing roofing materials on a new residential home located at 6021 Valley Quail, Seville, Ohio 44273. The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.
