29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   On or about May 2, 2019, during the installation of roofing material on a residential apartment complex located at 1846 Lincoln Way East, Massillon, Ohio 44646, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards and struck by hazards (employees working directly under roofing work) from working surfaces of varying heights approximately 8.1 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about May 2, 2019, during the installation of roofing material at a job site located at 1846 Lincoln Way East, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 8.1 feet by utilizing a fall protection system.
