Employees were performing shingles work on the roof of a two story residential home without adequate fall protection training.  An employee sustained serious injuries when he fell approximately 9 feet from the roof to the ground.  This condition exposed employees to fall hazards of approximately 9 feet.

The employees were accessing the roof to perform roofing work, and the ladder did not extend at least 3 feet above the surface being accessed nor was the ladder secured to a rigid support. This condition exposed employees to fall hazards from approximately 8 feet.
