29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.    On or about 2 May 2019, and at times prior thereto, a bench grinder abrasive wheel was missing the tongue guard allowing the gap between the wheel surface and the wheel housing to exceed 1/4th inch, exposing the employee using the grinder to the hazard of being struck by flying projectiles.
