29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    a) Within the plating department at 3601 South Clinton Avenue in South Plainfield NJ; employee(s) are required to wear half-face respirator for respiratory protection from formaldehyde gas during bi-weekly manual replenishment of electroless copper plating bath solution.  A respiratory protection program containing written procedures for respirator selection, fitting as well as provisions for employee training was not established and implemented.    Condition(s) were noted on or about 5/09/2019.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    a) Floor space was not kept clear in front of electrical equipment such as circuit breaker cabinet(s), located along the east wall at the back of plating department. Drum(s), box(es) and other materials stored directly in front of the electrical equipment impeded access for service, examination, etc.; exposing employee(s) to electrical hazard(s).
