29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Production area- On or about June 6, 2019 and at times prior, employees were exposed to a safety hazard in that, the employer had not established written machine specific procedures for employees performing maintenance such as, but not limited to, replacing parts on the heads of the Ward water jet machine and replacing bearings on slitter machines.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met: (a) Production area- On or about May 3, 2019 and at times prior, employees were exposed to a health hazard in that, the employer failed to establish a written hazard communication program for employees exposed to hazardous substances such as, but not limited to, machine hydraulic fluids, Simalfa 310 adhesive containing zinc oxide, and polyethylene foam containing polyethylene resins, iosbutane and talc.
