29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Optimizer 1 - On or about April 30, 2019 the employer did not provide adequate guarding for the saw blade on the Optimizer, exposing employees to amputation hazards.    (b) Optimizer 2 - On or about April 30, 2019 the employer did not provide adequate guarding for the saw blade on the Optimizer, exposing employees to amputation hazards.    (c) Optimizer 3 - On or about April 30, 2019 the employer did not provide adequate guarding for the saw blade on the Optimizer, exposing employees to amputation hazards.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  (a) Vacuum pumps outside main building - On or about April 30, 2019, the employer did not provide adequate guarding for the rotating drive shafts of two vacuum pumps were not guarded. Employees that clean and provide maintenance are exposed to being caught-in the unguarded rotating drive shafts.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition.     (a) Planer/Rip Facility North Wall - On or about April 30, 2019 the employer did not ensure aisles and passageway to critical equipment disconnects were kept clean and passable exposing employees to trip hazards when attempting to access the electrical panels for performing energy control procedures.      Industrial Timber, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(1)), which was contained in OSHA inspection number 1162197, citation number 01, item number 001 and was affirmed as a final order on September 07, 2016, with respect to a workplace located at 781 CR 549 in Ripley, MS 38663.
