Employees were performing masonry operations from the 2nd working platform, approximately 13 feet above ground level.    Observed, on or about 5/1/19.

29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one were not restrained from tipping by guying, tying, bracing, or equivalent means:  a) Jobsite: 1862 Delmar DR, Folcroft PA. 19032, Scaffold - The supported tubular welded frame scaffold, measuring approximately 20' 9" feet in total elevation, and 5' 1" feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. Employees were performing masonry operations from the 2nd working platform approximately 13 feet above ground level.  Observed on or about 5/1/19.

The employees were working from the 2nd working platform approximately 13 feet above ground level.  Observed on or about 5/1/19.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a) Jobsite: 1862 Delmar DR, Folcroft PA. 19032, Scaffold - The employer did not provide a safe means of access to the work platforms on the supported tubular welded frame scaffold.  Employees had to step-up greater than 2 feet to climb the scaffold supports to access the working platform levels.    Observed, on or about 5/1/19.

The employer did not ensure that the scaffold was erected under the supervision and direction of a competent person qualified to recognized and correct such hazards.   Noted on or about 5/1/19.
