29 CFR 1910.179(l)(2)(i)(d): Warning or "out of order" signs were not placed on the crane, also on the floor beneath or on the hook where visible from the floor: a) Facility, where cranes taken out of service and not equipped with signage included: 1. Carpenter Shop, where the annual inspection of the 1 Ton Coffing hoist supported on the unmarked overhead I-beam, indicated it was no longer suitable for use. (Item Corrected) 2. Third Power Plant, where the annual inspection of the gantry crane determined that there were kinks in the wire ropes.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Shops, where machinery not equipped with guarding included: 1. Maintenance Repair Shop, where the AAMCO brake lathe was not equipped with a guard. Hazard: Caught in rotating parts and struck by flying chips. 2. Sheet Metal Shop, where the Cincinnati metal shear was not equipped with guards to prohibit employees from entering into the space behind the shear. Hazard: Struck by pieces of metal. 3. Weld Shop, where the American Hercules metal shear was not equipped with guards to prohibit employees from entering into the space behind the shear and to protect employees working in the welding area or in the passageway adjacent to the shear. Hazard: Struck by pieces of metal.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: a) Main Machine Shop, where a spray bottle containing isopropyl alcohol was not equipped with a hazard warning.
