29 CFR 1926.403(a): Electrical conductors and equipment were not approved:  a. On or about May 03, 2019, at 1968 Lake Heritage Circle, near building 6, employees were using power supplied from extension cords connecting a 3 outlet adapter through the use of alligator clamps. The alligator clamps were directly connected to a circuit breaker and ground bar. The use of a homemade 2 conductor with alligator clip type connectors were not an approved electrical device.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  a.  On or about May 03, 2019, at 1968 Lake Heritage Circle, near building 6, employees were using power supplied from extension cords connecting a 3 outlet adapter through the use of alligator clamps. The alligator clamps were connected hot to hot and neutral to ground. There was no neutral path to complete the circuit.

On or about May 03, 2019, at 1968 Lake Heritage Circle, near building 6, employees had to reach inside an energized breaker panel to install alligator type clips onto energized conductors.

29 CFR 1926.404(b)(1)(ii): 1926.404(b)(1)(i): At this job site, the extension cords, being used to provide temporary power to energize portable power tools, were not protected with a ground fault circuit interrupter as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees from electrical shock hazards on this construction site:  a. On or about May 03, 2019, at 1968 Lake Heritage Circle, near building 6, employees were using power supplied from extension cords connecting a 3 outlet adapter through the use of alligator clamps and the alligator clamps were directly connected to a circuit breaker and ground bar. There was no ground fault circuit interrupter between the breaker panel, extension cord, and the hand tools being used.

Safety of equipment shall be determined on the basis of the following considerations:  (a)  On or about May 03, 2019, at 1968 Lake Heritage Circle, near building 6, the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees. Employees were using power supplied from extension cords that had not been inspected for damage prior to use.
