29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 3290 Sawmill Run Boulevard, Pittsburgh, PA: On or about May 3, 2019, during steel erection activities, the employer did not ensure that retraining was providing for employees using a Genie GS 1930 model scissor lift unit to weld steel in that employees were standing on and/or climbing on the guardrails of the scissor lift. b) Job site, 3290 Sawmill Run Boulevard, Pittsburgh, PA: On or about May 3, 2019, during steel erection activities, the employer did not ensure that retraining was providing for employees using a SkyJack SJ111-3219 model scissor lift unit to weld steel in that employees were standing on and/or climbing on the guardrails of the scissor lift.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Job site, 3290 Sawmill Run Boulevard, Pittsburgh, PA: On or about May 3, 2019, during steel erection activities, the employer did not ensure that a safe means of accessing the 2nd story was used in that an employee climbed onto a scaffold, a structural beam, a block wall, a concrete floor slab and through the 2nd story perimeter wire rope guardrails in order to get from ground level to the 2nd story.
