29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: On or about May 3, 2019, at the jobsite, an employee was exposed to a fall hazard of approximately 20 feet when installing laminate from a piece of plywood that was elevated using a JLG Skytrack rough terrain forklift without manufacturer approval for use.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about May 3, 2019, at the jobsite, an employee was working on a JLG Skytrack's platform at an approximate height of 20 feet without fall arrest systems, exposing the employee to fall hazards.
