Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  On and about May 6, 2019, employees were exposed to hazards associated with construction when the employer did not ensure PPE, including head, face, eye, hearing, and hand protection was available and used by employees exposed to struck by, noise, and laceration hazards, in that hardhats, safety glasses, gloves, and hearing protection were not provided or used.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).  On and about May 6, 2019, employees operating a Genie GTH-644 RT forklift were not trained to handle suspended loads and the employer did not certify training was done for any operators.

On and about May 6, 2019, employees were exposed to falls while using ladders to frame a house when the employer did not train each employee when ladders or stairs were necessary or how to properly select and use ladders, in that an employee was climbing an 8' step ladder that was leaned against the wall, and stairs or ladders were not used when the first floor deck was >19" above the garage floor and grade.

On and about May 6, 2019, employees were exposed to shock and burn hazards while working with power tools and extension cords that were damaged and/or improperly repaired, in that:    A: Three Skilsaw brand circular saws had cuts in the cords, some of which had been repaired with electrical tape;    B: One Skilsaw brand circular saw was missing the strain relief where the cord entered the tool body;    C: Two extension cords in use had unrepaired jacket damage and one had the cord jacket pulled loose from the plug.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On and about May 6, 2019, employees were exposed to fall hazards while framing a house in that:    A: The floor hole for the stairs to the basement was not protected by guardrails;    B: Wall openings where the fall height to the ground was >6' were not protected by guardrails;    C: A carpenter was sitting on the top plate of a wall 11'-6" above the ground without fall protection.

29 CFR 1926.1053(b)(4): Ladders shall be used only for the purpose for which they were designed.    On May 6, 2019, an employee was exposed to falling while accessing/egressing the top plate of a wall by climbing up and down an 8' fiberglass step ladder that was leaned against a wall.
