Employees did not control all hazardous energy sources such as, but not limited to, electrical before conducting activities such as, but not limited to, conducting roll change-overs. Employees were exposed to caught-in hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.

Employees were exposed to caught-in hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.
