29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    A)     Pexco Packaging Corporation, located at 795 Berdan Avenue, Toledo, OH 43610: On or about May 1, 2019, the employer did not develop and document specific procedures to control hazardous energy of the equipment such as, but not limited to, the FMC Flexographic Press # 1 (Model 586, Serial # 3646) located in the printing department. Employees did not control all hazardous energy sources such as, but not limited to, electrical before conducting activities such as, but not limited to, conducting roll change-overs. Employees were exposed to caught-in hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.

Employees did not deenergize the FMC Flexographic Press # 1 (Model 586, Serial # 3646), located in the printing department, prior to conducting activities such as, but not limited to, conducting roll change-overs. Employees were exposed to caught-in hazards that could result in personal injuries while all hazardous energy sources were not controlled prior to performing the tasks.
