(a) 854 Press, Model #8- 33 McGovern Boulevard, Crescent, PA: On or about April 12, 2019, the part revolution power press's brake failed causing the ram to fall and pin the operators hands between the ram and the dye set; the brake system monitoring was not so installed to properly indicate a fault in the braking system.
