29 CFR 1926.652(a)(1): Employees were not protected in an excavation from a cave-in by an adequate protective system designed in accordance with paragraph (b) and (c) of this section. Location: Veterans Administration Hospital, Building 3, 940 Belmont Street, Brockton, MA Walls of the trench were not shored or sloped to prevent cave-in while an employee was connecting a sewer pipe and spreading gravel in the trench. Pond View Excavation, Corp. was previously cited for a violation of this occupational safety and health standard 1926.652(a)(1), which was contained in OSHA inspection number 1298304, citation number 1, item number 1b and was affirmed as a final order on April 13, 2018, with respect to a workplace located at 372 Carpenter Street Providence, RI 02901.
