29 CFR 1910.134(c)(1): The employer did not establish and implement a written respiratory protection program with worksite-specific procedures in any workplace where respirators were necessary to protect the health of the employee or whenever respirators were required by the employer.   On or about May 7, 2019, and at times prior thereto, the employer did not ensure that a written respirator program was implemented. Employees are required to wear Elipse P100 half face respirators when performing welding inside a permit required confined space. Employee(s) are exposed to inhalation hazard.   (i)-Procedures for selecting respirators for use in the workplace;  (ii)-Medical evaluations of employees required to use respirators;  (iii)-Fit testing procedures for tight-fitting respirators;  (iv)-Procedures for proper use of respirators in routine and reasonably foreseeable emergency situations;  (v)-Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and otherwise maintaining respirators;  (vii)-Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;  (viii)-Training of employees in the proper use of respirators, including putting on and removing the, any limitations on their use, and their maintenance; and (xi)-Procedures for regularly evaluating the effectiveness of the program.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:  On or about May 7,  2019, and times prior thereto, the employer failed to ensure the internal conductors were not exposed at the strain relief area. Employees exposed to the hazard of an electrical shock. Such equipment as, but not limited too:  1. Delta Rockwell drill press, 110 volt,  the internal conductors were exposed at the strain relief area on the plug.  2. Two (2) welders called Millermatic 252, 240/480 volt, the internal conductors were exposed at strain relief area at the plugs.  3. Miller welder XMT 450, 230/460 volt, internal conductors were exposed at the strain relief area were the cord comes out of the frame on the miller welder.
