29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Worksite:  On or about 5/7/2019, the employer had not developed a Lockout/Tag-out program, including written procedures, nor had any training on the energy control procedure taken place with Authorized and Affected employees.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:     (a) Worksite:  On or about 5/7/2019, the employer utilized flexible cords as a substitute for fixed wiring by securing the cords to the floor with tape, wrapping one of the cords around a stanchion and securing the cord to the stanchion and another to the frame of a machine with wire-ties. A third flexible cord was strung through a pallet rack system to bring power from one aisle to another.
