29 CFR 1910.107(d)(2): Spraying area(s) were not provided with mechanical ventilation adequate to remove flammable vapors, mists, or powders to a safe location and to confine and control combustible residues: On April 27, 2019 at the worksite located at 1016 Progress Street, Mount Vernon, Missouri 65712, employees were exposed to serious fire and explosion hazards. A spraying area inside of a tank was not provided with mechanical ventilation adequate to remove flammable vapors, mists, or powders to a safe location and to confine and control combustible residues. The lack of mechanical ventilation caused flammable vapors to ignite when an employee was conducting welding operations.
